When making your appointment...

To better serve our patients and minimize your wait time, we request you make separate appointments for each family member to be seen by the doctor

We suggest you arrive 15 minutes before your scheduled time to allow our staff to process your paperwork in a timely manner.


Our physicians want to address all of your medical concerns. In order to adequately address all concerns, we must schedule the appropriate amount of time. When scheduling an appointment, please:


  • Share with the scheduler the reason for your visit.
  • Schedule separate appointments for multiple concerns.

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. We ask that you give us 24 hours in advance notice providing an opportunity for another patient to been seen.

Please bring the following to your appointment

To insure insurance claims are being processed correctly and we are up to date on any changes in your treatment plans being administered by another provider, please bring the following items to each visit:


  • Current insurance card(s)
  • Photo ID
  • All current medications and dosing instructions
  • Over-the-counter supplements or vitamins
  • Co-payment
